NPI Lookup

Enter a 10-digit NPI to see the registered name, individual or organization type, and NPPES status. The lookup also shows any Medicare revalidation records in our CMS dataset. Free, with no login required.

Free lookup

Find a provider by NPI.

Identity comes from the public NPPES registry. Revalidation results come from the CMS Medicare Revalidation Due Date List; the source date is shown when available.

An active NPI or a revalidation record does not confirm current Medicare enrollment, licensure, credentialing or payer participation. A missing revalidation record does not mean the provider is not enrolled.

We check the CMS national registry (NPPES) and the Medicare Revalidation Due Date List (PECOS). Enter the provider or organization's 10-digit NPI — no login needed.

Understanding your result

NPI lookup, answered.

What is an NPI number?

An NPI is a 10-digit identifier assigned through NPPES to eligible healthcare providers who apply. Type 1 identifies an individual and Type 2 identifies an organization. It does not certify professional qualifications. Read CMS guidance on NPIs.

Who needs an NPI?

Healthcare providers covered by HIPAA must obtain an NPI for use in HIPAA standard transactions. Other eligible providers may apply too. See CMS eligibility and NPI requirements.

What information does this tool display?

The NPPES result shows the name, NPI, individual or organization type, and NPPES record status. When revalidation records are available, the table shows the enrollment ID, specialty, program, state and listed revalidation due date. This tool does not display the NPPES practice address or taxonomy. Those are available in the official registry.

Does a result confirm Medicare enrollment?

No. NPPES status concerns the NPI record. The revalidation dataset shows published revalidation records, not a real-time enrollment decision. A missing record does not establish whether a provider is enrolled. Verify enrollment through PECOS or the provider's Medicare Administrative Contractor; start with CMS enrollment guidance.

What should I do if I cannot find a record?

Check the NPI with the provider, then search the official NPI Registry or CMS Revalidation List. Read the source date alongside any results. Providers manage NPI information in NPPES. For help planning an enrollment application, explore our Medicare enrollment services.
